1. What is it with ～？で「～はどういうわけなんだ？」という意味になります。この場面では、Whereを使ってWhere did / does all this cigarette smoke come from?のようにも言えます。
2. Howとkeep from ～ ing「～しないでいる」を組み合わせて「どうやったら気分が悪くならないのか」と言っているのです。wonder「驚き、不思議」を用いてIt's a wonder it doesn't make you sick.のように表現してもいいでしょう。
3. この場合のlastは「耐える、もちこたえる」の意味になります。「窒息する」をsuffocateの代わりにchoke to death「息が詰まって死ぬ」を使って表現しています。I'd choke to death in about ten minutes in here.とも言えます。
