join「参加する、加わる」。「本当に~でいいの(確かなの)?」をAre you sure ~?で表現。この場合のthatは「そんなに」という意味の副詞。don't have to be ~で「~でなくてもいい」の意味。extra「臨時の、余分な」、help out「手を貸す、手伝う」。
